Setting up your profile and completing intake forms

Having some troubles figuring out the online booking? Can’t find where to fill out intake forms? See below for some help!

You’ve received a welcome e-mail from the clinic, if you’re not seeing it please make sure to check your spam folder, if there’s still no e-mail please contact the clinic to make sure your information is entered correctly. You can also head over to https://alignedwellchiro.janeapp.com/admin

First, start by setting up your account.

Next, choose your prefered method. Remember your sign-in as you’ll need it to book future appointments.

Fill out your information, make sure you fill out all REQUIRED information or you will not be able to proceed.

Once you’ve entered all your information you now have access to your account! From here you can manage appointments, fill out intake forms if needed, change your information, make accounts for family members.

If you created an account without booking an appointment the needed intake forms will be available after booking as shown in the picture.
